Volunteer Application

You can send in your application alone, but if you want to speed up the process sending the documents below filled out and completed would significantly speed things up.          

Volunteer Forms that will need to be completed prior to start

  1. Criminal-records-release-
    1. Answers for CRR form:
      1.  Fill in name and address and sign
      2. Sign in front of Notary or call to meet with our Notary.
  2. BEA form
    1. Answers for BEA form:
      1.  We will complete this portion
      2. Complete and sign and have a friend witness it and sign
  3. DMV – Release of Motor Vehicle Records  
    1. Answers For DMV Form:
      1. Answer 1.  B.
      2.  Answer 2. we fill out
      3.  Check box for Driver record (certified copy)
      4. 4. Leave blank
      5. Fill in name and address and license.
      6. Sign in front of Notary or call to meet with our Notary.
  4. Medical Release
    1. Have your doctor sign and return it to us
  5. New Volunteer Agreement
    1. This form is also for current drivers updating their information.
  6. Online Safe driver Course– (FREE)
    1. FIRST TIME USER REGISTER: Link  Volunteer Driver Safety Course Link
    2. Click the “ADD TO CART
    3. In the upper right hand corner, click the “VIEW CART”
    4. On Cart Page, Enter“SUPERVISOR” email address INFO@READYRIDES.ORG; Enter (1) quantity in “Quantity Box” for “DRIVER” taking course then Go to the bottom right corner and click on the ‘CHECK OUT” box.
    5. On Contact Information Page, Enter “DRIVER” email address and the “DRIVER” name (same as driver’s license) company address and phone number under billing address.
    6. Balance will read ZERO proceed to Click “CONTINUE PAYMENT
    7. On this page, verify the “DRIVER” information is correct; The “Payment Box” will read “NO PAYMENT IS REQUIRED”
    8. Go to the bottom of the page, click “PAY NOW
    9. Go to the Upper left-hand corner click “START COURSE” to continue registration process
    10. On the “WELCOME” page. The screen will show driver email address, first and last name. And you must create your own PASSWORD in the “PASSWORD” box.
    11. Go to the bottom of the page, click “REGISTER AND VIEW PURCHASE”.
    12. Go to the upper right hand and corner click the “START” button to begin the course. When course is finished it will send us a certificate and you can print your own too, also good to show insurance companies to try to lower your insurance.

Title-VI-Complaint-Form Ready-Rides

Ready Rides gives notice of its policy to assure full compliance with Title VI of the Civil Rights Act of 1964 and all related statues. Title VI requires that no person in the United States of America shall, on the grounds of race, color, or national origin, be excluded from participation in, be denied the benefits of, or be otherwise subjected to discrimination under any program or activity for which Ready Rides receives Federal Financial Assistance. To request a copy of Ready Rides Title VI Program, contact Ready Rides Title VI Coordinator. 

Any person who believes that he or she has, individually, or as a member of any group, been excluded from participation in, been denied the benefits of, or otherwise subjected to discrimination under any Ready Rides service, program or activity, and believes discrimination is based upon race, color, or national origin has the right to file a formal complaint. 

Complaints will be accepted in writing and may be filed with Ready Rides Title VI Coordinator at the address below. A signed written complaint must be submitted within 180 days of the alleged discriminatory act (or latest occurrence). Individuals may also file complaints directly with the US Department of Transportation (USDOT), and/or the Federal Transit Administration (FTA) within the 180-day timeframe. Complaints filed directly with USDOT and or FTA are transit related complaints. 

Complaints should include, at a minimum, the following information: 

  • Your name, address, and a telephone number where you can be reached during business hours; 
  • A general description of the person(s) injured by the alleged discriminatory act(s); 
  • A description of the alleged discriminatory act(s) in sufficient detail to enable the Title VI Coordinator to understand what occurred, when it occurred, and the basis of the alleged discrimination regarding race, color, or national origin. 
  • The letter must be signed and dated by the person filing the complaint or by someone authorized to do so on his or her behalf.
